Family as a social institution Essay

Social institution is a set system with values, roles, norms and statutes organized in a manner that satisfies societal needs. One major example of a social institution is the family which is defined as a group of people related by blood, marriage or adoption who lives together and share economic resources. American families are mostly organized in form of nuclear family which consists of father, mother and children. Extended family is another form of family arrangement recognized by Americans and consists of uncles, aunts and grand parents. The family structure has changed in various aspects that are related to certain social factors. Andrew J. Cherlin in his book argues that marriage in America has taken new direction different from other developed and developing nations. Marriage which is a bond that leads to existence of a family is faced with a lot of conflicts such as same sex marriage. One major social aspect that has led to changes in family structure is adoption of different cultural ideals. Family is considered as a means of formal commitment through which individuals share life with one another but this is taking another shape because of different ideologies. Through marriage, an aspect of individualism is emphasized by personal choice which is a form of self development that tries to make a family live as one unit. Nuclear type of family is mostly recognized in U. S but growth of population and adoption of different cultural practices has led to changes in family structure. The social aspect of population growth and its impact on family structure can be attributed with migration of family members from overseas countries. Individuals move from other countries to U. S in search of better jobs or to further their education. Once they are in America, such individuals are hosted by family members such as uncles or aunts making the structure of family to change. A family that consisted of husband, wife and children massively expands making its structure to change. Thus population growth and migration of individuals is attributed with changes in family organization or structure. Adoption of cultural practices through intermarriage is another social factor that has led to changes in family structure. In the recent past, individuals from different cultures marry. This allows people to adopt certain cultural practices whereby the role of husband, wife and children changes. Every individual in a family organization has his or her role to play as is defined by the Holy Book but nowadays changes have been adopted in defining the role of each family member. Civilization is a social factor that has led to changes in the structure of family. Most families in developing nations have adopted the live of western countries making the structure of family to change. The role of the family as a social institution Family plays important role in respect to certain responsibilities on children and the entire society as a whole. A family is a social institution that observed certain values, norms or statutes with an objective of existing as one unit. One major role of family in relation to its function on children is to provide the basic needs. Children in a family constitute people below the age of 18 years. Such individuals are not in a position to find money on their own and thus depend on parents to provide for them basic needs. Basic needs in this aspect consist of food, shelter and clothing which should be provided by husband and wife. Another basic role of family to children is enforcement of norms such as discipline, ethical considerations, moral values and strong base on religion. Parents in a family are supposed to provide strong discipline to their children so as to have a bright future. Discipline is one of the core things a family cannot deny its children because it is the beginning of wisdom and adoption of good moral values. Parents should teach their children about religion when they are young so as to have a strong base in Christianity, Islamic teachings, Hinduism and any other form of religion recognized by the constitution. Parents in a family are very instrumental in the aspect of teaching children how to behave and be responsible adults. Family as a social institution plays a very important role concerning the entire society. A society is a collection of many families with common goal and objective of sharing economic resources. Family members play a very important role through provision of needed services to serve the society. The parents work in various sectors of the economy thus they participate through collective responsibility. Certain services that are provided by the parents include teaching, nursing, accounting, engineering and legal services. All these services are needed by other members in the society and thus it avails services required by the entire community. The family engages in various social activities in the society whereby the children participate in schools, churches games, competition and entire society. Disc Arthroplasty Essay

Degenerative disc disease have been a problem for many people as this disease brings about low back pain that seems to paralyze the working force,   the middle adulthood population (Smeltzer, 2004). Not only does it bring impaired quality of life to those who suffer from it, but Arvind Kulkarni adds large health care expenses to the list of problems brought about by this disease as seen in his article entitled Prosthetic Lumbar disc replacement for degenerative disc disease. Spinal fusion, which is the medical management for degenerative disc disease, has been criticized by several doctors in the practice due to its untoward effects in the patient’s overall recovery and lifetime improvement. Specialist in the field, Christoph Siepe, identified these unpleasant effects as accelerated adjacent system degeneration, pseudarthrosis, spinal canal stenosis, and donor site morbidity. Adjacent segment degeneration, as mentioned by Peter Ullrich in his article entitled Fusion versus Artificial Disc Arthroplasty for Lumbar Degenerative Disc Disease, stated that this is the major complication of spinal fusion and the reason why most surgeons want this procedure replaced. Adjacent segment degeneration or ASD happens when mechanical stress is transferred to the adjacent segment, also mentioned by Peter Ullrich. But research showed that this procedure can be modified to show more positive results. An anterior lumbar interbody fusion showed a low occurrence of revision surgery needed, which meant low frequency of adjacent segment degeneration, all pointed out in Mr. Ullrich’s paper.   However, to achieve this outcome, Mr. Ullrich declared that there should be an improvement in patient selection and in the procedure. The patients should be those patients having degenerative disc disease in their 5th lumbar to 1st sacral segment, because there is minimal motion at this level. By doing so, he states that fusing these segments do not change the patient’s mobility as much.   But not all are satisfied of this result. Doctors and specialists are continuously making efforts to totally changing the primary medical management for degenerative disc diseases. Studies have been conducted, and several replacement procedures were approved by the FDA. Peter Ullrich stated in his paper that the total disk replacement procedure was approved for medical use by the FDA in the year 2004 using the Charite artificial disc but for only one level of the spinal vertebrae, being the L4 – L5 or the L5 – S1. This is not the first time that a total or partial disc replacement was done. The history of this procedure was mentioned in an article in a website by Spine-Health that disc nucleus replacement was done first in 1955 by David Cleveland by injecting methyl-acrylic. Ever since then, there have two other FDA approved products aside from the Charite, the website Spine-Health states that the Prodisc was approved on August 2006 and the Kineflex lumbar on June 2005.   Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚   A total disc replacement is being described by Jose Reyna in his article, Advances in Artificial Disc Technology, as being composed of two pieces of plates made of metal that has teeth to be able to attach itself to the end plates of the vertebral bones above and below it. This means that the particular intervertebral disc that is causing pain has to be removed and replaced by an artificial disc. Several artificial discs have been developed by various manufacturers in the country and they only differ in design and composition. The Charite, as mentioned earlier, is made up of a unique sliding core with two metal alloy end plates. The Prodisc being based on spherical articulations is made up of cobalt chromium molybdenum alloy as stated in the website Spine-Health. Inventors Qi-Bin Bao and Paul Higham developed a hydrogel prosthetic nucleus made up of PVA powder and a solvent mixed together, as seen on the article, Hydrogel intervertebral disc nucleus with diminished lateral bulging. The total disc replacement procedure has its advantages and disadvantages. Even after some models being approved by the FDA, it is still being assessed thoroughly by specialists in the field. Charles Rosen and Douglas Kiester report poor results and complications in the total disc replacement procedure. As it was mentioned in their study, this procedure causes fractures, severe pain, dislocations, extrusion of the implant, facet joint degeneration, and unexplained radiculopathy. They attribute these substandard results to the fact that the center of rotation of segmented motion is erroneous. Jose Reyna Jr. also mentions several disadvantages of the procedure in his study, stating that the anterior approach to the surgery can cause injury to the aorta and the common iliac vessels, can cause retrograde ejaculation in men by injuring the superior hypogastric plexus, hemorrhage can result from the retroperitoneal approach, and infection can occur. However, he also stated some advantages, as this procedure is said to maintain range of motion in majority of the patients as a long-term outcome. There are still a lot of studies to be conducted, and patients to be experimented on. A three year study conducted by Christoph Siepe et al., entitled Clinical Results of Total Lumbar Disc Replacement With ProDisc II: Three-Year Results for Different Indications, intended to assess total disc replacement outcome in patients who actually underwent the operation. Their results showed that not all degenerative disc diseases can be treated with this medical management. The best outcome was seen in patients exhibiting degenerative disc disease condition plus soft disc herniation or nucleus pulposus prolapse. Outcome was measured against the patient’s improvement from their preoperative levels. There were inferior results from the patients who had bisegmental damages, and a higher complication rate. Also it was mentioned that the younger patients, more specifically those aged below forty exhibited better outcome than those older than forty. Technology like this is still being developed, studied and modified as we speak. What is important is that there are people out there who care and who try to make a difference, whether it is by inventing, criticizing or being the experimental person wherein these new devices will be made a reality. The process of this would take into account the scanning of external and internal environmental factors, long term and strategic planning for strategy formulation as well as the implementation and evaluation and control process. Wheelen Hunger(2006,3) strongly focused on the analyzing and evaluating of external opportunities and threats compared to the companys strengths and weaknesses. A SWOT (Strength, Weakness, Opportunities and Threats) analysis is an ideal tool for strategic managers to plan for both predictable and unfeasible contingencies. In the point of Dess and Miller(1993), the combination of three major interrelated activities; strategic analysis, strategic formulation and strategy implementation can be called strategic management. These activities are a continuous process that helps evaluates and controls the company and the industry it is in, benchmarks the competition and set realistic targets and strategies to meet its existing and potential competition. Lamb(1984) states that strategic management is a continuous process that evaluates and controls the business and industries the company is in; it also assesses the competitors to set goals and strategies then reevaluates them every quarterly or annually to decide on the implementation of the strategy is successful. It then reevaluates the if there is any new factors such as technology, competition, economic market, social, financial and even political environment that can be used to create a new strategy. As a summary, Strategic Management and Planning is the understanding of the process of strategy formulation, implementation and evaluation. For company an organization to gain the edge over its competition regardless of size, scale; each organization needs to have a well planned strategic management in order to strive and compete in the market while optimizing its potential for tomorrows trends of today. With four new customers every second, the latest figures exceed, yet again, all industry predictions and demonstrate the persistently dramatic growth of GSM throughout the world. As at the end of 1997, there were more than 200 million mobile phone subscribers worldwide and the global sales volume was over 100 million units, an increase of about 50% from the previous year. By the end of the year 2001, it is believed that the global subscriber base will have grown to approximately 600 million. Coupled with the subscriber increase, the mobile phone sales volumes are growing due to the rise of the replacement market. GSM has established itself as the worlds leading digital wireless standard, and it is believed that the current growth rates will continue for the foreseeable future. At present levels, it is confident in the predictions that by the year 2005, it will have achieved between 700 million and One Billion GSM customers worldwide. The GSM industry is one of the most impressive success stories of the 1990s and currently generates more than $100 Billion a year in subscriber revenues alone. Its impressive growth rates are once again attributed to soaring global markets and the sustained penetration of new emerging markets. In all the markets it has been starting to see a widespread take-up of a host of GSM value added services, beyond voice usage, including data and text applications. This demonstrates how GSM is an evolving technology that offers exciting new services and opportunities for subscribers. The future is even more exciting, as the development path for the GSM standard is clearly mapped out and presents an exciting era of applications and services that will stretch the bounds of the imagination. It is moving closer and closer to the delivery of third generation services, which will see multi-media applications, internet access and access to real-time video, via GSM-based networks and handsets. This will ensure that GSM continues to be the standard of choice and heralds continued massive customer growth in the years to come. The Emirates Telecommunication Corporation Etisalat: The UAE has a prominent status in the field of telecommunications as its achievements are reaching the quality and standards of the developed countries. The demands for telecommunications facilities in the UAE are growing at a phenomenal rate. The local Emirates Telecommunications Corporation (ETISALAT) which is 60% state-owned controls this sector and 40% owned by private UAE investors. Since its creation in 1976, Etisalat has increased the number of telephone lines from 50,000 to over 850,000 and plans to raise switching capacity by an annual rate of 100,000. In the UAE, demand for new lines is growing at about 12% per year. As far as mobile phones are concerned, the number of subscriptions reached 1.2 million in end of 1999 compared to 128,495 in 1995. As it is roughly one-in-two of the population and 30,000 to 40,000 new phones were being sold each month. Thus Etisalat maintained its leading position among the countries of the region by realizing the highest growth rates of mobile phones at 50 phones for every 100 people. Fax Plus: A new fax plus state-of-the-art service with a host of advanced facilities which lets the person operate even without owning a fax machine. Paging Service: This radio paging service enables people on the move to be in touch in an easy and economical way almost anywhere in the UAE. According to Al Ittihad Newspaper, the UAE is ranked 7th worldwide, with 10% of the population using this service. Voice Mail Service: This includes videoconference service, consultation services, and interactive information service. ATM Service: The Asynchronous Transfer Mode (ATM) is a new service launched recently in order to speed up connections to customers. This is a broadband communications technology, which provides a way of transporting data from a wide range of applications using one seamless network. This service is being introduced to cater for the growing needs of education and business in the UAE and as part of Etisalats planning for the future. Satellite A new satellite company called Al-Thurayya Satellite Communications Organization was launched in January 1997. Its capital is worth $25 million and owned 26% by Etisalat, 20% by the Abu Dhabi Investments Company, 10% by the Arab Satellite Communications Organization (Arabsat), and 10% by Bahrain Telecommunications Company (Batelco), and 44% is owned by other Arab government. The company was set up to own and operates the Thurayya satellite system, which will serve the rapidly expanding mobile phone sector. Operators now face the challenges of rising customer quantity, the prospect of declining revenue per subscriber, and the scrutiny of performance by investors. Achieving success in these new market conditions calls for a change in strategy from mobile operators a fresh approach to marketing, distribution and customer service to attract more new customers whilst retaining existing customers and protecting future revenue. The buyers understand the concept of service and support, and are much more likely to pay for it when the offering is clearly stated. There is no doubt that we compete much more against all the box pushers than against other service providers. I have some valuable advice for you on how to make this transition smoother and an enjoyable experience rather than a scary and lonely one. My number one piece of advice is for you to enter this experience with an open mind. If you come into this new situation with a good outlook and a positive attitude, your experience will be a lot more rewarding. Now that you are on your own and have very limited restrictions, it can be easy to slip into bad habits that could get you into trouble. Here you do not have a parent to tell you when to go to bed, or when to study, or not to go out, or not to party, or to remind you to go to class. It is all up to you and your self-control. You must have great self-discipline and a motivated attitude in order to succeed. You have to want to be here and make the best of your experience. This one being your first year in college, you may have trouble concentrating while you study. Some techniques that can help your concentration are being selective, relaxing, and over learning. Being selective means to make choices about what is most important to learn. When you are studying, you should be able to know what may or may not be on the test, your notes for the week can be helpful. In addition, pay attention to charts, tables, and illustrations that you think may appear on the test. Relaxing is a good technique to use because when you are relaxed you are able to absorb more information. One way to relax is to use a breathing technique (inhale, count from one to ten, and exhale, and count from one to ten, and so on). Over learning can help you fight mental vagueness. When you are struggling in a class, it is best for you to learn more than you need to know about the subject. You can use these techniques every time you are studying or when you cannot concentrate. Contract and United Airlines

Cardillo Travel Systems, Inc. ACT 1 Russell Smith knew why he had been summoned to the office of A. Walter Rognlien, the 74-year-old chairman of the board and chief executive officer (CEO) of Smith’s employer, Cardillo Travel Systems, Inc. Just two days earlier, Cardillo’s in-house attorney, Raymond Riley, had requested that Smith, the company’s controller, sign an affidavit regarding the nature of a transaction Rognlien had negotiated with the United Airlines.The affidavit stated that the transaction involves $203,000 payment by United Airlines to Cardillo but failed to disclose why the payment was being made or for what specific purpose the funds would be used. The affidavit included a statement indicating that Cardillo’s stockholders’ equity exceeded $3 million, a statement that Smith knew to be incorrect. Smith also knew that Cardillo was involved in a lawsuit and that court injunction issued in the case required the company to maintain stockhol ders’ equity of at least $3 million.Because of the blatant misrepresentation in the affidavit concerning Cardillo’s stockholders’ equity and a sense of uneasiness regarding United Airlines’ payment to Cardillo, Smith had refused to sign the affidavit. When Smith stepped into Rognlien’s office on that day in May 1985, he found not only Rognlien but also Riley and two other Cardillo executives. One of the other executives was Esther Lawrence, the firm’s energetic 44-year-old persistent and chief operating officer (COO) and Rognlien’s wife and confidante. Lawrence, a long-time employee, had assumed control of Cardillo’s day-to-day operations in 1948.Rognlien’s two sons by a previous marriage had left the company in the early 1980s following a power struggle with Lawrence and their father. As Smith sat waiting for the meeting to begin, his apprehension mounted. Although Cardillo had a long and proud history, in recent years the company had begun experiencing serious financial problems. Founded in 1935 and purchased in 1956 by Rognlien, Cardillo ranked as the fourth-largest company in the travel agency industry and was the first to be listed on a national stock exchange. Cardillo’s annual revenues had steadily increased after Rognlien acquired the company, approaching $100 million by 1984.Unfortunately, the company’s operating expenses had increased more rapidly. Between 1982 and 1984, Cardillo posted collective losses of nearly $1. 5 million. These poor operating results were largely due to an aggressive franchising strategy implemented by Rognlien. In 1984 alone that strategy more than doubled the number of travel agency franchises operated by Cardillo. Shortly after the meeting began, the overbearing and volatile Rognlien demanded that Smith sign the affidavit. When Smith steadfastly refused, Rognlien showed him the first page of an unsigned agreement between United Airlines and Cardill o.Rognlien then explained that the $203,000 payment was intended to cover expenses incurred by Cardillo in changing from American Airlines’ Apollo system. Although the payment was intended to reimburse Cardillo for those expenses and was refundable to United Airlines if not spent, Rognlien wanted Smith to record the payment immediately as revenue. Not surprisingly, Roglien’s suggested treatment of the United Airlines payment would allow Cardillo to meet the $3 million minimum stockholders’ equity threshold established by the court order outstanding against the company.Without hesitation, Smith informed Rognlien that recognizing the United Airlines payment as revenue would be improper. At that point, â€Å"Rognlien told Smith that he was incompetent and unprofessional because he refused to book the united payment as income. Rognlien further told Smith that Cardillo did not need a controller like Smith who would not do what was expected of him†. ACT 2 In No vember 1985, Helen Shepherd, the audit partner supervising the 1985 audit of Cardillo by Touche Ross, stumbled across information in the client’s files regarding the agreement Rognlien had negotiated with United Airlines earlier that year.When Shepherd asked her subordinates about this agreement, one of them told her of a $ 203,000 adjusting entry Cardillo had recorded in late June. That entry, which follows, had been approved by Lawrence and was apparently linked to the United Airlines-Cardillo transaction: Dr ReceivablesUnited Airlines$203,210 Cr Travel Commissions and Fees203,210 Shepherd’s subordinates had discovered the adjusting entry during their second-quarter review of Cardillo’s form 10-Q statement. When asked, Lawrence explanation without attempting to corroborate it with other audit evidence.After discussing the adjusting entry with her subordinates, Shepherd questioned Lawrence. Lawrence insisted that the adjusting entry had been properly recorded. Shepherd than requested that Lawrence asks United Airlines to provide Touch Ross with a confirmation verifying the key stipulations of the agreement with Cardillo. Shepherd’s concern regarding the adjusting entry stemmed from information she had reviewed in the client’s files that the United Airlines payment to Cardillo was refundable under certain conditions and thus not recognizable immediately as revenue.Shortly after the meeting between Shepherd and Lawrence, Walter Rognlien contacted the audit partner. Like Lawrence, Rognlien maintained that the $203,000 amount had been properly recorded as commission revenue during the second quarter. Rognlien also told Shepherd that the disputed amount, which United Airlines paid to Cardillo during the third quarter of 1985, was not refundable to United Airlines under any circumstances. After some prodding by Shepherd, Rognlien agreed to allow her to request a confirmation from United Airlines concerning certain features of the agreement.Shepherd received the requested confirmation from United Airlines on December 17, 1986. The confirmation stated that the disputed amount was refundable through 1990 if certain stipulations of the contractual agreement between the two parties were not fulfilled. After receiving the confirmation, Shepherd called Rognlien and asked him to explain the obvious difference of opinion between United Airlines and Cardillo regarding the terms of their agreement with the chairman of the board of United Airlines. â€Å"Rognlien claimed that pursuant to this confidential business arrangement, the $203,210 would never have to repaid the United.Shepherd’s conversation with Rognlien refused. In fact, as Rognlien knew, no such agreement existed. † A few days following Shepherd’s conversation with Rognlien, she advised William Kaye, Cardillo’s vice president of finance, that the $203,000 amount could not be recognized as revenue until the contractual agreement wi th United Airlines expired in 1990. Kaye refused to make the appropriate adjusting entry, explaining that Lawrence had insisted that the payment from United Airlines be credited to a revenue account. On December 30, 1958, Rognlien called Shepherd and told her that he was terminating Cardillo’s relationship with Touche Ross.In early February 1986, Cardillo filled a form 8-K statement with the Securities and Exchange Commission (SEC) notifying that agency of the company’s change in auditors. SEC regulations required Cardillo to disclose in the 8-K statement any disagreements involving accounting, auditing, or financial reporting issues with its former auditor. The 8-K, signed by Lawrence, indicated that no such disagreements preceded Cardillo’s decision to dismiss Touche Ross. SEC regulations also required Touche Ross to draft a letter commenting on the existence of any disagreements with Cardillo.This letter had to be filed as an exhibit to the 8-K statement. In touche Ross’s exhibit letter, Shepherd discussed that the improper accounting treatment given that transaction resulted in misrepresented financial statements for Cardillo for the six months ended June 30, 1985, and the nine months ended September 30, 1985. In late February 1986, Raymond Riley, Cardillo’s legal counsel, wrote Shepherd and insisted that she had misinterpreted the United Airlines-Cardillo transaction in the Touch Ross exhibit letter filed with the company’s 8-K.Riley also informed Shepherd that Cardillo would not pay the $17,500 invoice that Touche Ross had submitted to his company. This invoice was for professional services Touche Ross had rendered prior to being dismissed by Rognlien. ACT 3 On January 21, 1986, Cardillo retained KMG Main Hurdman (KMG) to replace Touche Ross as its independent audit firm. KMG soon addressed the accounting treatment Cardillo had applied to the United Airlines payment. When KMG personnel discussed the payment with Rognlien, he informed them to the alleged secret arrangement with United Airlines that superseded the written contractual agreement.According to Rognlien, the secret arrangement precluded United Airlines from demanding a refund of the $203,000 payment under any circumstances. KMG refused to accept this explanation. Roger Shlonsky, the KMG audit partner responsible for Cardillo engagement, told Rognlien that the payment would have to be recognized as revenue on a pro rata basis over the five-year period of the written contractual agreement with United Airlines. Cardillo began experiencing severe liquidity problems in early 1986. These problems worsened a few months later when a judge imposed a $685,000 judgment on Cardillo to resolve a civil suit filed against the company.Following the judge? s ruling Raymond Riley alerted Rognlien and Lawrence that the adverse judgment qualified as a â€Å"material event† and thus has to be reported to the SEC in a Form 8-K filling. In the me morandum he sent to his superiors, Riley discussed the serious implications of not disclosing the settlement to the SEC: â€Å"My primary concern by not releasing such report and information is that the officers and directors of Cardillo may be subject to violation of rule 10b-5 of the SEC rules by failing to disclose information that may be material to a potential investor. Within 10 days of receiving Riley’s memorandum, Rognlien sold 100,000 shares of Cardillo stock in the open market. Two weeks later, Lawrence issued a press release disclosing for the first time the adverse legal settlement or that Cardillo remained viable only because Rognlien had invested in the company the proceeds from the sale of the 100,000 shares of stock. Additionally, Lawrence’s press release, Roger Shlonsky met with Rognlien and Lawrence. Shlonsky informed them that the press released grossly understated Cardillo’s estimated loss for fiscal 1985. Shortly after that meeting, KMG res igned as Cardillo’s independent audit firm.EPILOGUE In May 1987, the creditors of Cardillo Travel Systems, Inc. forced the company into involuntary bankruptcy proceedings. Later that same year, the SEC concluded a lengthy investigation of the firm. The SEC found that Rognlien, Lawrence, and Kaye had violated several provisions of the federal securities laws. These violations included making false representations to outside auditors, failing to maintain accurate financial records, and failing to file prompt financial reports with the SEC, In addition, the federal agency charged Rognlien with violating the insider trading provisions of the federal securities laws.As a result of these findings, the SEC imposed permanent injunctions on each of the three individuals that prohibit them from engaging in future violations of federal securities laws. The SEC also attempted to recover from Rognlien the $237,000 he received from selling the 100,000 shares of Cardillo stock in April 1986 .
